I just tried to do this quest with me, another player on the quest, and someone not on the quest helping us. I was the party leader. We did Shrouded Mauler (me and the other player on the quest spawned it), neither of us died, our friend who isn't on the quest killed it. We go back upstairs, I walk out of the building and forget to talk to the orange guy, whereas my friend on the quest remembers and he gets his note. I go back inside, try to get the note, and he just gives me the "Something stirs in the depths below..." message.

I'm not sure what this is caused by, but that's the most information I can provide.

Yes, I did. He asked me to help him stop the monsters down below like he usually does.

I've had a theory that if you don't actually directly damage the mauler he may ignore your participation.

Every time I make sure I hit the mauler, I get credit. I have yet to test it however.

I guess that's a possibility. I can't remember for sure but I'm pretty sure there's been times where I've spawned the mauler and then run away and let whoever I was with do all the damage.

Unlikely, in my opinion, but I suppose its plausible.

I remember hearing it that he has to "acknowledge" you with his AI, as in check your aggro. But that was years ago.
