When I died, I had been meeting the prof requirements to level for a while, but I hadnt gained the prof to actulay cause me to level. Upon death I lost proficiencies and gained alevel at the same.

I dont know if it matters much since I heard you were removing prof loss upon death except for cube deaths, but it seemd woth enough to report.
